- Letter of commitment from the student to incorporate principles of high-value care into both their education and their future career (submitted to site mentors)

Students interested in receiving the letter of distinction may apply to the STARS site mentor via email ([email protected]). Please include the activities completed and dates of completion of the above four criteria in your application in a MSWord document titled “Lastname_Firstname STARS HVC LoD 2022”. Incomplete applications which do not meet all four criteria will not be considered. Final approval will be subject to the site mentor reviewing and approving the student’s application (in consultation with the panel of co-mentors Dr. Ilene Rosenberg and Dr. Howard A. Selinger) and forwarding to the Senior Associate Dean of Education.
We look forward to hearing back from you.
Sincerely,
Dr. Anand
